I hereby agree to cooperate and participate in a research project entitled: Safe Inking: Internet InterventionTo Decrease the Risks of Disease Transmission Associated With Body Art, to be conducted by Louise Komorek, BSN student SIUE, as principal investigator. It is my understanding that:

 All of the research data will be collected & analyzed in a manner that assures my confidentiality.

  As a respondent, I will be asked only to read the teaching material at "Safe Inking" and to complete and return the survey.

  There are no experimental procedures used in this study, and no known risks from participation.

  I may ask questions of the researcher & expect pertinent responses.  I may contact the researcher by email at safeinking@cs.com at any time during this study. I may expect a reply within a maximum time allowance of 48 hours.

  I may refuse to participate in the study or may discontinue participation at any time without prejudice, question, or reprimand.

  The benefits of this study include possible increased knowledge regarding the avoidance of disease exposure for study participants. There are no known risks to study participants.
